To receive FARMA Blood Transfusion Kits, you need:

  1. Fill out this Google Form:

  2. Answer some clarifying questions, which you will probably receive afterwards upon contacts indicated in the form;
  3. You will have to wait for some time if there is no stock of ready-made kits.

Currently, the kits are produced in limited batches and it is therefore particularly important that the available quantities end up where they will be used safely and effectively. Therefore, the main criteria for the distribution of kits are the availability of appropriate medical qualifications of the staff and actual experience in blood transfusion.

Please note that Blood Transfusion Kits are not a replacement for a proper blood chain, but only a back-up option in case of complications with getting blood products in harsh or disaster conditions.

The project is not commercial, so you cannot purchase the kits. If you would like to thank the team for their work and continue the project, we invite you to make a donation which will make us able to create even more kits that will be provided to the armed forces free of charge.

Reporting

Currently, there is no need to sign any documents to achieve full field version of kit, but a photo report is required, as described below:

  1. A photo immediately after receiving the shipment – so that you can see you with the box and the emergency sticker on it;
  2. A photo in a working environment, so that we may see the kits themselves and the surroundings of the location (stabilization center, medevac vehicle, etc.);
  3. A photo during use on a patient or during training (depending on which type of kit you order).

    Please, cover the faces of the participants, if necessary, in any convenient way.

Photo-reports are important to support the project and ensure that we can continue to manufacture and supply kits for the needs of our medical services. Please take this request responsibly.

For simplified training versions of kit we will expect among the above to receive official written request and signed invoice upon receiving, as training version assembled by the sponsors support and requires official reporting.
